Assignment Form

I, LastName, FirstName (“Customer”), hereby assign all my claims against the operating airline regarding the flight disruption on the above mentioned flight arising out of Passenger Rights EU Regulation 261/2004, the Montreal Convention 1999 (MC99), UK Air Passenger Rights Law (UK261), the Brazilian Consumer Code and Brazilian Aeronautical Code, Turkish "SHY" passenger regulation, Canadian Transportation Act: Air Passenger Protection Regulations (SOR-2019-150) to Click 2 Refund Inc, and Click2Refund Inc. hereby accepts the assignment.

I declare that I have read and agreed the Terms & Conditions and I have not assigned the Claim to a third party. I authorize Click2Refund Inc. to request the operating carrier not to process my personal data in relation to the Claim pursuant to applicable personal data protection laws, except only to verify the Claim.

I understand that this means I cannot accept any direct contact or payment from the operating carrier. If the assignment pursuant to this assignment form is declared invalid for any reason, the assignment form shall be considered as a power of attorney granted by me to Click 2 Refund Inc., pursuant to which Click 2 Refund Inc. is granted exclusive power, with full substitution right, to:

• represent me legally before third parties in relation to the Claim;
• obtain every type of information required, initiate civil or administrative requests and complaints;
• initiate negotiations and take legal or extrajudicial measures to collect my Claim;
• collect and receive payments in relation to the Claim on my behalf.
